LPN Intake Specialist
Location: West Palm Beach
Salary Range: $35.00-$36.00 per hour
Full Time: M-F 9:00am –5:30pm
Matrix Homecare is seeking an experienced LPN Intake Specialist to join our team functioning as a
liaison to referral sources and family by coordinating referrals and facilitating the provision of home care
services and care management. This position reports to the Director of Corporate Services.
Duties and Responsibilities:
1. Receives and or coordinates Central Intake referrals, which may include
discussions with insurance providers, physicians, and family members.
2. Take physician orders and documents.
3. Assists prospective clients and referral sources in identifying appropriate services
to meet the client’s home care needs.
4. Provides referral sources with Company scope of service, coverage area, etc.
5. Where referrals require a coordination of care to offer catastrophic care, and
work as a team member with the clinical management team.
6. Enters referral information into Company EMR software.
Qualifications
1. Currently licensed as an LPN in the state of Florida. The license must not have been revoked,
suspended, and without limitations or restrictions.
2. Minimum of three years health care experience, intake case management and/or home health
experience preferred.
3. Knowledge of Medicare, Medicaid, private insurance, and catastrophic care.
4. .Bi-Lingual Spanish/English Preferred
Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes,
including fingerprinting and review for eligibility before employment.

Full-Time Clinical Nursing Supervisor Home Health Agency
Registered Nurse (RN) Field Nurse
Location: South Palm Beach Area (Boca Raton & Delray Beach)
Salary Range: $80,000 – $84,000 per year based on experience
Key Responsibilities:
Perform in-home visits to provide nursing assessments, medication management, and care coordination for private and Medicare clients. Administer wound care and IV infusion therapy as necessary.
Collaborate with physicians, caregivers, and other healthcare professionals to develop and update care plans. Ensure comprehensive documentation and accurate reporting of patient progress and outcomes. Provide education to patients and their families on managing their health conditions at home. Travel throughout the Boca Raton and Delray Beach areas as part of regular home visits. Be available for at least one weekend per month for patient care coverage
Position Overview:
We are seeking a compassionate and experienced Field Registered Nurse (RN) to join our dedicated team as Clinical Nursing Supervisor, serving the South Palm Beach area, including Boca Raton and Delray Beach. This full-time position requires providing high-quality, patient-centered care to both private and Medicare clients in their homes. The ideal candidate will have at least two years of home health care experience, with a strong background in wound care and IV infusion therapy
Requirements:
- Current Florida RN license in good standing.
- Minimum of 2 years of Home Health care experience, including in-home visits to private and
Medicare clients. - Proven experience in wound care and IV infusion therapy.
Strong communication and interpersonal skills, with a focus on patient care and family
education. - Ability to work independently, manage time effectively, and stay organized while traveling
throughout the service area. - Spanish-speaking skills are a plus.
Position Details:
Full-time, Monday to Friday (with at least 1 weekend per month required).
Salary: $80,000 – $84,000 per year, depending on experience.
Travel required within the South Palm Beach area (Boca Raton & Delray Beach).
Supervision of Field Staff to include clinical and technology oversight
Development of the skills of the staff as required to service the patients.
